The 4th Dimension

Back in Time

It is possible with the 4D-Facelift!

Introduction – the 4th Dimension

Since 2017, we truly understand how all the pieces of the puzzle fit together.

With 22 years of experience in facelift surgeries, over 50 scientific articles, more than 75 presentations at conferences, and more than 4500 facial surgeries, we can now say that thanks to ‘cause-based thinking’ the circle is complete.

If the cause of facial changes is due to sagging (1D), skin damage (2D), and volume loss (3D), then the best approach is to address all three aspects simultaneously!

Results have been achieved like never before.

Just as this woman feels after losing 89 kg and feeling fit and strong again in her mind, she now looks the same on the outside! Younger, fitter without a surgical appearance.

Read her personal and disarming story here.

“… The best comment came from someone who was in my class in high school: when I see you, I see you as you were when we were in class together.”

Where do you start with the most comprehensive facial treatment ever shown on Dutch TV?!

Follow Carla after she previously lost 89 kg and now has her aged face treated with a brow lift, upper and lower eyelid surgery, neck and face lift, correction of turkey neck bands, lipofilling, PRP, and a TCA peel, all at the same time.

Examples of the 4D-Facelift

After progressing from student to teacher, there is an impressive series of class photos showing that if you look 61 before the procedure, you can look 43 at the age of 62!

Detailed recordings

With special photo-measuring equipment, the 4D-Facelift shows unparalleled improvement in skin quality and deeper wrinkles, including around the mouth and cheeks.

When we let the computer score the wrinkles, it shows an improvement of at least 50% fewer fine lines in the epidermis.

Even damage to blood vessels can heal through our special method of improving skin quality, both on the surface and below, at the cellular level, through regeneration.

Read more about this at PRPSclinics.com, where this technique is also used for repairing joint and scar damage.

With our special ‘double approach’ to the skin, both from the inside-out and from the outside-in, the texture of the epidermis also measurably improves!

Risks

Thanks to our many years of experience and well-trained team, there are few problems. We can quickly identify and solve minor issues.

Anyone considering surgery should be well-prepared and aware of potential problems. Always make sure you are well-informed during the consultation and ensure all your questions are answered. Below you will find more information.

General risks

General risks for any surgery include bleeding and infection.

  • Infection is very rare unless there is a general reduction in immunity. Smoking increases this risk. It is explicitly expected that you do NOT SMOKE for at least 6 weeks before the procedure and for at least 2 weeks after the procedure.
  • Bleeding is also very rare. However, the risk is significantly increased if you are in a high-risk group (smokers, poorly controlled high blood pressure, use of blood thinners, and other medications that thin the blood). In such cases, it is always advised to first schedule a preoperative consultation with the treating specialists and to wait with a neck-face lift until you are out of the high-risk area.
  • If a large hematoma occurs immediately after the procedure, an additional procedure may sometimes be necessary to close a leaking blood vessel. To address this quickly and adequately, one overnight stay is recommended. This way, such an inconvenience, if it occurs, never leads to a permanent reduction in the final result. Although some bruising can occur, within 2 to 4 weeks after surgery, the person is almost always presentable again.
  • Dehiscence is the separation of the skin edges at the scar site. Specifically for neck-face lifts, it should be emphasized that the risk of this is greatly increased if the person has not stopped smoking in time. The result can be an unsightly separating wound, especially behind the ear, which can take several weeks to fully close. Wound care is not difficult (shower 1-2 times a day, dress the wound with greasy gauze – Unitulle – twice a day). The result is usually a somewhat widened and retracted scar. From 9 months after surgery, a scar correction can be performed. This is also done without additional costs in the mentioned circumstances. It is better to prevent this by completely stopping smoking at least 6 weeks before surgery.
  • If you are taking medication or have had other issues after previous surgeries, you should always report this during the preoperative consultation to take it into account if necessary.
  • For procedures performed on both sides, such as a neck-face lift, absolute symmetry cannot be guaranteed. Usually, there was no absolute symmetry before the procedure either, as the two halves of the face are never exactly the same.
  • The scar in front of the ear fades so quickly that after 2 weeks, only a minimal line remains, which lies in a natural crease at the front edge of the ear. However, behind the ear, the skin is tougher and thicker, so the scar will remain stiff, thicker, red, and wide for longer. This is normal and not a risk in itself. However, it can cause prolonged discomfort if you do not consider your hairstyle. During this period, tightly pulled-up hair is not advisable. With slightly longer hair directly behind the ear, you can camouflage the scar. Post-treatment with silicone-containing scar cream is recommended.
  • Note: Neck-face lifts in men generally cause more immediate postoperative discomfort because:
  • Men generally do not like to use facial camouflage to cover the scars during the first 3 months when the scar may still be somewhat red.
  • With a technique introduced in the Netherlands by Dr. Jeroen Stevens, this surgery is possible for men. The scar can be completely hidden in the groove behind the ear.
  • Some risks that theoretically exist but are very rare include temporary fluid accumulation, which can sometimes be removed with a puncture (a small needle) if the swelling becomes too uncomfortable. The risk of facial nerve damage is extremely low with the technique used by Dr. Stevens (lateral SMASectomy) and has not occurred in his practice to date. If this happens, such discomfort is generally temporary (6 to 9 months).
  • Changes in sensation occur with any procedure where the skin is lifted and tightened (abdominoplasty, forehead lift, etc.). Usually, it involves a small area in front of the ear or near the earlobe where the skin may feel temporarily dull.
  • Changes in skin sensation: After any procedure involving an incision in the skin, the sensation of that skin often feels temporarily different. Often dull, sometimes (temporarily) more sensitive, tingling, stinging, or burning. Although it almost always eventually feels nearly normal again, it can take longer than you would like (sometimes 3-6 months to sometimes 1 1/2 years). Sometimes the sensation can be really bothersome. Desensitization exercises may then be applicable.

FAQ

Costs of the various treatments?

In general, your insurance company will not cover the costs for lipofilling and/or a facelift.

Click here for the prices applicable at Velthuis Clinic Rotterdam for the aforementioned procedures. Note: all amounts are indicative and subject to change.

Unfortunately, this currently also applies to PRPS. Click here for information on relocating your repair system at the cellular level and the costs if you cannot get rid of joint pain or are bothered by a scar.

After an initial consultation, a personalized treatment plan can be provided with an accurate price quote. To make an outpatient appointment, click here.

What is the waiting time for the procedure to take place?

Generally, the surgery can take place within 4 to 6 weeks after your confirmation, provided you do not smoke. If you do smoke, ideally, the procedure can be scheduled 6 weeks after you have completely quit. Further arrangements can be made with the secretariat.

What can I expect from the results?

The aforementioned procedures have become well-predictable and highly rewarding after years of development. However, keep in mind that surgery is a very personal choice that may not always be understood by everyone. The most important thing is how you feel about it and what you think. The goal is to achieve harmony between how you feel inside and how you look outside.

Is the result permanent?

The aging of skin, muscles, and connective tissue (fortunately) continues. After 5 to 10 years, sometimes only after 15 years, there comes a time when the tissues start to sag again due to the loss of elasticity and the effect of gravity pulling downwards. How quickly this happens in your case depends on the quality of your skin, your overall health, sun exposure, and whether or not you smoke.

Are these procedures very painful?

No, a bruised feeling during the first few days is common. Some numbness in the operated area may also occur for a short period. Both are mainly due to the sensory nerves adjusting to the new, tighter position. This sensation is not painful, but it can be somewhat uncomfortable. Generally, if there is place for swelling, there is hardly any real pain. Of course, you will always receive adequate pain medication for the first few days after any procedure. For more detailed information, always refer to the relevant chapters.

Conclusion

If you are wrongly told that you look tired, even though you feel good, active, fit, and energetic and want to reflect that…

If you see more lines in your face than you think match how you feel, and you realize it’s not just a bit of sagging, and you recognize damage in the epidermis and volume changes…

Then a 4D-Facelift might be the solution for you.

The procedure is not extremely taxing and provides an impressive long-term improvement, depending on the quality of your skin, age, and lifestyle.
